Introduction[edit | edit source]
Long term conditions are defined as physical and mental health condition that require a long term management over a long period of time such as Diabetes, Parkison’s Disease[1]. They are also defined as chronic diseases or Non-communicable diseases (NCDs).
Examples:
- Heart disease
- Stroke
- Cancer
- Diabetes
- Chronic lung disease
They are responsible for almost 70% of all deaths around the globe and have devastating effects on the individual and society.
