I work as a Band 6 physiotherapist in Intermediate care since 2009, prior to this; I worked as a senior physiotherapist in an Orthopaedic centre. I have been qualified since 1997.In my current work place I am responsible for providing highly specialist assessment and treatment for patients in the community who have been discharged from the acute sector, or have been identified as needing physiotherapy input as part of an integrated, multi-professional and multi-agency intermediate care service. It’s my responsibility to ensure that pathways of care and communications are maintained with all agencies working with the service. As part of my role, key-working is undertaken. This involves checking that all multi-disciplinary needs are being met, also organising care planning alongside social services and other external agencies. Discharge planning needs to be completed throughout a patient transition through a pathway within the service. The key-worker role often requires me as a physiotherapist to identify additional support where required and refer to reablement services for a temporary package of care, in view of a long term care needs assessment. Often referrals to a care manager or active case manager are made to support the patient with long term needs.

As a senior clinician my key responsibility is to undertake a comprehensive assessment of patients receiving intermediate care including those with diverse or complex presentations/multi-pathologies, in order to identify their problems and needs and implement agreed treatment plans that respect people’s privacy, dignity and individuality. I provide high standard of clinical care for the patients under my management, and support other staff to do likewise; I assess, plan, implement and evaluate rehabilitation interventions for service users with complex presentations/multiple pathologies referred to the service, using advanced analytical and clinical reasoning skills.

As a part of my work I do train, clinically supervise and assist in the caseload management of Band 5 Physiotherapists, rehabilitation assistants, students coming for placement and care staff towards the management of patients, including clinically demanding patients.

As a training co-ordinator to my service I do actively participate in the planning and implementation of in-service training programme, by conducting Training needs analysis, audit and organising training by external agency. I ensure treatments offered to patients are based on the best available clinical evidence. I am responsible for teaching and training of trainee student physiotherapist. This involves teaching analysis and interpretation, clinical reasoning, supervision and marking of case scenarios, audits and projects. I attend regular journal clubs, seminars, courses, online readings, conference, special interest groups and meetings.
