Bed Recommendations
Introduction
The average person spends around one third of their lifetimes asleep.[1] Sleep directly impacts patients' health and mood and life quality.[2] Unfortunately, the prevalence of sleep disorders has shown a steep increase over the years.[3] [4] [5][6] Perhaps particularly relevant to the physiotherapy context is the instrumental role of sleep from a healing standpoint, essential for musculoskeletal injuries.
One way of enhancing sleep quality may lie in mattress choices for patients. Mattress choices can influence lower back pain. Patients with chronic lower-back pain appear to exhibit increased sensitivity to mattresses firmness.[7]A population-based study centered around schoolkids and their parents found strong associations between lower back pain in bed and upon rising with having lower back pain.[8]
Mattress Firmness
There is some evidence that medium firmness mattresses can be effective in reducing lower back pain and improving sleep quality.[9] [10][11] In the literature, mattress firmness has usually been assessed subjectively.[11]
Studies suggest medium firmness mattresses are superior to firm mattresses in reducing lower back pain in the daytime, for lying in bed, and upon rising.[12][13] Medium firm mattresses are also associated with a greater improvement in disability versus firm mattresses.[12] 'Medium firmness' was generally subjectively identified in the research, and one study suggested patients who adjusted and inflated their mattresses themselves had optimal sleep quality.[14]
Temperature Regulation
Some studies have looked at the effect of temperature on sleep quality. Temperature regulation is a key factor for sleep as a reduction in core body temperature triggers sleep, and conversely an increase in temperature promotes wakefulness.[15] It has been recommended that during sleep, skin temperatures should stay within 33.5–35.5 °C, as deviations from this range reduce total duration of sleep and deep sleep, due to an increase in sleep disturbances.[16]Maintaining ideal temperature also positively impacts perceived sleep quality.[17]
Clinical Recommendations
Overall, it appears that medium firmness mattresses may be beneficial in improving lower back pain and improving sleep quality. Temperature regulation, whether with temperature-controlled mattresses or air conditioning systems may also help with sleep quality.
