A prescription is a written, verbal, or electronic order from a practitioner or designated agent for a particular treatment for a specific patient.

The prescription should be always on the letterhead of the doctor

It should mention the doctor’s name, the address of the clinic, the educational degree of the doctor, registration details,

contact details and logo.
