Introduction

A prescription is a written, verbal, or electronic order from a practitioner or designated agent for a particular treatment for a specific patient.

The prescription should be always on the letterhead of the doctor

It should mention the doctor’s name, the address of the clinic, the educational degree of the doctor, registration details,

contact details and logo.

COMPONENTS OF A THERAPY PRESCRIPTION

Identifying Information

Including the patient’s name, date of birth, medical record numbers, etc.… along with the date the prescription was written.

Discipline

Indicate physical therapy (PT) or occupational therapy(OT). other disciplines includes athletic training, exercise physiology,

massage therapy, recreational therapy, even speech and language pathology.
